Please contact me via email or phone to make or change an appointment. In your message, please let me know the reason for the cancellation, and suggest times for rescheduling.
Note that appointments cancelled with less than 24 hours’ notice will be billed at the full rate.
If you are a new patient, feel free to contact me by email.
Please include days and times when you could attend an initial consultation, and a brief description of why you are asking for help. I will be back in touch to confirm an appointment. Please fill out and print the attached form and bring it to the initial consultation.
